Is Protein Powder Safe? What Indian Testing Actually Found

The short answer: a well-made protein powder is safe for most healthy adults - but "well-made" is doing a lot of work in that sentence. When independent researchers tested 36 protein supplements sold in India, around 70% did not contain the protein their labels claimed, and most carried detectable lead. The risk is not protein - it is the product.

Kavita, 46, came to me after her orthopaedist mentioned low muscle mass and suggested she "add protein." She is a vegetarian who had been eating what she considered a healthy diet: poha or upma for breakfast, dal-chawal-sabzi for lunch, khichdi at night. Her intake worked out to roughly 32 g of protein a day against a body that needed about 50 g. Her daughter had bought her a protein powder. Her sister had warned her it would ruin her kidneys. She had left the tub unopened for four months.

Kavita's kidneys were completely normal. The warning she had received applies to people who already have kidney disease - not to her. Meanwhile, four months of "being careful" had cost her four months of muscle she could not afford to lose.

Why This Question Is Different in India

Most articles answering "is protein powder safe" are written for a reader who eats plenty of protein already. That is not the typical Indian reader.

Indian diets are built on cereals - rice, wheat, millets - which carry protein, but less of it and in a less complete amino acid profile than animal foods or soy. The ICMR-NIN recommended intake for an average healthy Indian adult is around 0.83 g per kg of body weight per day: roughly 50 g for a 60 kg woman, 58 g for a 70 kg man. In consultations, vegetarian clients frequently land in the 30-40 g range without realising it, because dal-chawal feels like a protein meal when most of the plate is the rice.

So the question that matters is not just whether powder is dangerous. It is whether you have a real deficit to solve, and whether the product in front of you is honest about what is in it. That second question is where India has a genuine problem.

What Independent Testing Found in Indian Protein Powders

In 2024, a self-funded group of researchers published the Citizens Protein Project in the journal Medicine (Philips et al., DOI 10.1097/MD.0000000000037724). They bought 36 popular protein supplements from the Indian market and ran them through protein quantification, heavy metal testing, pesticide screening, aflatoxin detection and steroid analysis.

The findings, as reported in that paper:

  • Around 70% (25 of 36) were mislabelled on protein content - the measured protein was lower than the label claimed, in one case by more than 60%
  • Lead was detected in about 75% of samples, cadmium in roughly 28%
  • Aflatoxins (fungal toxins) in about 14%, pesticide residue in about 8%
  • Several products contained herbal additives with no clear benefit and, in the authors' assessment, potential for liver harm

A follow-up study by the same group, published in 2025, compared powders marketed as nutraceuticals against those marketed as pharmaceutical or medical-nutrition products, and found the pharma-marketed category performed considerably worse on both label accuracy and protein content.

Three honest caveats before you throw out your tub. This was an independent, self-funded study of 36 products - not an FSSAI regulatory audit, and 36 brands is not the whole Indian market. Detection of a heavy metal is not the same as a dose that will harm you; the concern with lead and cadmium is cumulative exposure, particularly for daily long-term users. And some well-made products did test accurately.

The Kidney Myth: Who Is Actually at Risk

This is the single most repeated piece of misinformation I encounter, and it keeps people like Kavita from eating enough.

In adults with normal kidney function, higher protein intakes within sensible ranges have not been shown to cause kidney damage. Protein increases the filtration workload, which shows up on tests as a normal physiological adaptation, not as injury.

The warning is real for a specific group: people who already have chronic kidney disease or reduced kidney function. In that situation, high protein intake can accelerate decline, and protein needs to be deliberately moderated under medical supervision - which is why our kidney diet guide exists as a separate resource.

The practical takeaway: if your kidney function is normal on a recent report, protein is not your risk. If you have kidney disease, diabetes with kidney involvement, or you do not know your numbers, get a creatinine and eGFR done and let your doctor set your protein target before you add any supplement.

Why Whey Upsets So Many Indian Stomachs

If whey gives you gas, bloating or loose motions, you are probably reacting to lactose rather than to protein itself.

Lactose malabsorption is substantially more common in South Asian adults than in Northern European populations, and whey is a dairy fraction - so how much lactose reaches your gut depends on which form you buy:

  • Whey concentrate - cheapest, roughly 70-80% protein, retains the most lactose. The usual culprit behind the symptoms.
  • Whey isolate - more filtered, roughly 90% protein, very little lactose. Usually tolerated by people who react to concentrate.
  • Whey hydrolysate - pre-digested, most expensive, rarely necessary for general use.
  • Plant blends - soy, pea, rice. Lactose-free by definition. Pea and rice are commonly blended because each is low in different amino acids and the combination covers both; soy isolate is complete on its own.

Who Actually Needs Protein Powder - and Who Is Wasting Money

This practice earns nothing from what you decide here - there is no product to steer you toward. So, the unvarnished version.

Powder helps:

  • Older adults losing muscle. After 50, muscle loss accelerates and appetite often shrinks. Getting 25-30 g of protein into someone who cannot finish a meal is exactly what a supplement is for.
  • People doing real resistance training - four or more sessions a week with progressive load, not a walk and ten minutes of stretching.
  • Vegetarians with a genuine, measured gap who have already maximised dal, curd, paneer, soy and sprouts and still fall short.
  • Recovery situations - post-surgery, post-illness, or during medically supervised rapid weight loss, including on GLP-1 medications where muscle loss is a known concern.
  • Very low appetite with high needs, where food volume is the barrier.

Powder is usually money wasted:

  • Sedentary or lightly active people buying it "for health" while already meeting their needs from food
  • Anyone adding it on top of an already adequate diet, in the belief that more protein is automatically better
  • People using it to replace meals - you lose the fibre, micronutrients and satiety of real food, and typically eat more later
  • Anyone buying an unverified product at premium prices, given what the testing above found

The honest summary for most vegetarian Indian adults with moderate activity: your protein target is reachable through food if you restructure the plate - more dal, curd, paneer, soy chunks, sprouts, and eggs if you eat them. Powder is a convenience, not a requirement. It becomes worth considering when food alone has failed, not before you have tried.

What a Gram of Protein Costs: Powder vs Your Kitchen

Prices vary by city, brand and season - these are approximate 2026 figures, meant for the ratio rather than the rupee:

Soy chunks deliver protein at roughly one-tenth the cost of whey isolate. That does not make powder pointless - it is faster, more portable, and easier to swallow when your appetite is poor - but it does mean "I cannot afford enough protein" is almost never true in an Indian kitchen. It is usually a planning problem.

How to Read an Indian Protein Label

Given the mislabelling findings, treat the label as a claim to be audited:

  1. Check the FSSAI licence number on the pack. No number, no purchase.
  2. Read protein per 100 g, not per scoop. A 40 g scoop delivering 24 g protein is a 60% protein powder - respectable, but not the "80%" the front of the tub implies.
  3. Add up the amino acid claims. Products can be "spiked" with cheap free amino acids like glycine or taurine that inflate measured nitrogen without delivering usable protein. Long amino acid lists near the top of the ingredients deserve suspicion.
  4. Look for third-party testing - Informed Choice, NSF Certified for Sport, or a published batch certificate of analysis. This is the single most useful signal available to a consumer.
  5. Check added sugar, especially in "mass gainer" and flavoured variants.
  6. Be sceptical of herbal add-ons. Ashwagandha, shilajit and similar inclusions are marketing in most protein products, and the Citizens Protein Project authors raised concern about liver-harm potential from some herbal additives.
  7. Prefer plain over proprietary blends. "Proprietary blend" means the manufacturer will not tell you the ratio.

If you have liver or kidney disease, are pregnant, or take regular medication, speak to your doctor before adding any supplement - that is a medical decision, not a shopping one.

If You Are Buying Protein Powder Outside India

The protein question follows Indians abroad, and the answers shift.

Supplement regulation is looser than most people assume, everywhere. In the United States, dietary supplements are not approved by the FDA before sale - manufacturers are responsible for their own safety and labelling claims, with enforcement acting largely after problems appear. The UK and EU regulate more tightly, but protein powders are still food supplements rather than medicines. The Indian findings above are stark, but they are not evidence that products abroad are automatically clean: independent testing in the United States has repeatedly found heavy metals in protein powders too, and that testing is what prompted much of the current scrutiny.

Third-party certification is far easier to find abroad, and it is the single most useful signal. Look for NSF Certified for Sport or Informed Sport / Informed Choice on the tub. These schemes test batches independently, and they are common on products sold in the USA, UK, Canada and Australia - considerably more so than in India. If you have the option, buying a certified product is the clearest protection available to you as a buyer.

Be careful with Indian brands bought abroad or carried from India. Many NRI households stock Indian-manufactured powders, either from a South Asian grocer or in a suitcase after a visit home. Those products sit squarely within the market the Citizens Protein Project sampled - so the label-auditing habits above apply to them in full, and more so than to a certified product bought locally.

The cost comparison changes but the conclusion does not. Whey is relatively cheaper abroad than in India and lentils relatively dearer, so the gap narrows - but dal, soy chunks, paneer, eggs and milk remain considerably cheaper per gram of protein than powder in the USA, UK, Canada and the Gulf. Powder still buys convenience rather than nutrition you cannot otherwise get.

Your vegetarian options are better than they are in India. Calcium-fortified soy milk, tofu in ordinary supermarkets, Greek yoghurt, cottage cheese, canned beans, and a far wider range of plant protein isolates are all easy to find abroad. Many NRI clients discover that the protein gap they arrived with is easier to close in a Western supermarket than it was at home - once they stop treating dal-chawal as the whole answer.

One thing that does not change: if you have kidney disease, are pregnant, or take regular medication, the decision to add a supplement belongs with your doctor, wherever you live.
